A. About two weeks after enrollment you will receive a membership kit containing your United Discount Dental Card, a list of providers in your area and a detailed description of the discounts available. Simply show your card at participating provider locations at the time of your appointment and your adjustment will be granted at the time you receive the products or services.

A. No. There are no claim forms or other paperwork to file. Most providers will provide you with the network price immediately during your visit, unless they prefer to have a re-priced bill mailed to both the member and provider.

A. There are no limitations on the use of the program, regardless of your health condition. There are no annual or lifetime maximums on medical use.

Q. What happens once I sign up?
A. You will receive your membership kit within 10-14 business days after your initial sign-up. Your enrollment pack will include your United Discount Dental membership cards, which will list you and any dependents that you include in your program together with a comprehensive list of providers. When your cards arrive, you can immediately use the program at your doctors, dentist and pharmacy, according to its terms and conditions, by presenting your card to the health care providers listed under the program.

A. Your United Discount Dental membership is valid anywhere in the United States. Just call us toll-free and we will supply you with provider names closest to your geographic area.

A. For all preventive dentistry there is a fixed fee schedule every plan dentist has agreed to use. You will receive this schedule with your membeship kit.

A. No. Unlike insurance, Qualified Dental has no restrictions, exclusions and no physical is required. Once your application is processed, you will receive a membership packet containing your Health Card and an instruction booklet. Your card will indicate all of your necessary information.

A. Each provider in our system has a contract with their provider network. The relevant PPO Networks, Effective Date and a toll-free Provider Relations Number will all be on the membership card in order to assist the provider to process your visit at the reduced rate.

Q. How is the United Discount Dental Card different from insurance?
A. The United Discount Dental Card is NOT insurance!  We offer savings for dental-related services. There are no claim forms to complete.

Q. I already have insurance. How does the United Discount Dental Card work with my insurance?
A. United Discount Dental can be used with some insurance to reduce the amount of out-of-pocket expenses. The United Discount Dental savings are based on the total cost of the service/product before any insurance claims are filed. Therefore, if your insurance has a high deductible or a high co-payment, the United Discount Dental savings can be applied to help reduce some of the out-of-pocket expense.  You may be able to use your card for prescription drugs not covered by your HMO or managed care plan. You can also use the Card to cover expenses not covered by most insurance plans such as dental and chiropractic care.  However, United Discount Dental does not undertake to pay for or reimburse you for your dental care expenses.
